The injured employee was standing on a flat bed truck reading labels on parts to another employee who was on the ground. The injured employee put their foot on an interior support column weighing 826 pounds that was stacked four high and the stack shifted. The employee jumped to the ground and the column fell on top of him. The employee was hospitalized for broken ribs, a punctured lung, a punctured liver and internal bleeding.

When the employee placed a piece of metal into the shear, the weight difference between the heavy end and lighter end caused the piece of metal to be raised above the shear table. The shear cycle then activated via the foot pedal, and the hold-downs and guard lowered and forced the piece of metal down toward the shear table. The employee's right index fingertip was pinched and amputated between the metal and the shear table.
